CLAIMS MAY BE CHALLENGED. PERSONS THAT FILE FALSE OR FRAUDLENT CLAIMS MAY BE PROSECUTED. 1. □ I certify that I am or was a Comcast High-Speed Internet Service account holder who has/had Comcast High-Speed Internet Service at the address(es) listed above; and 2. I certify that, as a Comcast High-Speed Internet Service account holder, □ I used or attempted to use the Comcast service to share computer files using one of the “Peer-to-Peer” protocol(s) listed below (you must check all of the “Peer-to-Peer” protocols listed below that apply) at some time from April 1, 2006 to December 31, 2008, and that (i) I was unable to share such files using Comcast service; and/or (ii) I have reason to believe that the speed at which files were shared was impaired while using the Comcast service; □ Ares □ BitTorrent □ eDonkey □ FastTrack □ Gnutella □ I attempted but was unable to use the Comcast service to send emails using Lotus Notes at some time from March 26, 2007 to October 3, 2007; and 3. □ I certify that I have not previously requested or received a refund on or credit to my account arising out of or related to Comcast’s management of P2P file-sharing traffic or Lotus Notes on its high-speed Internet network or Comcast’s disclosure of its congestion management practices; and 4. □ I certify that my use or attempted use of the “Peer-to-Peer” protocol(s) specified above to share computer files and/or Lotus Notes to send emails was for a lawful purpose consistent with applicable copyright and other laws. |
